About Us
Divine Innovation Services, LLC (DIS) is a licensed North Carolina home care agency dedicated to empowering the individuals and families we serve to reach their full potential and live confidently in their communities. We provide person-centered support to individuals with mental health, developmental disabilities, and substance abuse service needs (MH/DD/SAS) across North Carolina.
As a contracted Tailored Care provider with Alliance Health, Trillium Health Resources, and Vaya Health, we deliver direct services — including Community Living and Supports, Respite, Supported Living, and Individual Transitional Support — that meet North Carolina Medicaid standards and the expectations of the Managed Care Organizations we partner with.
